Liability Release And Assumption Of Risks Agreement

By voluntarily participating in any way in the garbage collection, cleaning and program of activities related to LIMPIAR PUERTO RICO INC., the person signing this document acknowledges, concedes and agrees that: with your participation you recognize the risk of being hurt in the execution of the voluntary work of these activities and that, although remote, the risk includes the possibility of permanent paralysis and death, and that although the use of rules, equipment and discipline may reduce the risk, the risk of injury always exists. ACKNOWLEDGING THESE RISKS, I FREELY ASSUME THE RISKS AND TOTAL RESPONSIBILITY FOR THE RISKS OVER MY PARTICIPATION, known and unknown, EVEN IF THESE RISKS HAPPEN FOR THE NEGLIGENCE OF THE PARTY BEING RELEASED FROM RESPONSIBILITY, or of any other person. I freely agree to abide by the terms and conditions determined for my voluntary participation in the program, but if for any reason during my participation I observe a significant and unusual danger, I will refrain from participating and will immediately bring this situation to the attention of an officer. I, and on behalf of my heirs, assignees, personal representatives and close relatives, BY THIS MEANS I RELEASE AND DISCHARGE LIMPIAR PUERTO RICO INC. FROM ALL CLAIMS, LIABILITY AND RESPONSIBILITY, including its officers, directors, volunteers, agents, employees, other participants, sponsors, agencies of marketing, publicists, and if applicable, owners and lessors of the properties to be used to conduct the event, FOR ANY AND ALL INJURY, DISABILITY, DEATH, loss or damage to persons or property, IF PRESENTED BY NEGLIGENCE OF THE PARTY BEING RELEASED FROM RESPONSIBILITY OR OTHERWISE.

I CERTIFY THAT I HAVE READ THIS LIABILITY RELEASE AND ASSUMPTION OF RISKS AGREEMENT, I FULLY UNDERSTAND ITS TERMS, I UNDERSTAND THAT I HAVE RENOUNCED SUBSTANTIAL RIGHTS BY SIGNING IT, AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY MOTIVATION OR PRESSURE.

MINOR PARTICIPANTS (UNDER 18 YEARS OF AGE AT TIME OF REGISTRATION)

I certify that, as the parent/guardian with legal responsibility over this participant, I consent to and agree to your release of liability as set forth above, for all relieved of liability, and for myself, assignees, my heirs, and close relatives. I relieve LIMPIAR PUERTO RICO INC., and agree to indemnify and exonerate them from any and all incidents of my minor relative during their participation or involvement in these programs as set forth above, EVEN IF PRESENTED BY THE NEGLIGENCE OF THE PARTY BEING RELEASED FROM RESPONSIBILITY, to the maximum extent permitted by law.
